CANADIAN CUSTOMER NOTICE
All customers in Canada must read and agree to this form in order to be eligible to place orders for products. Please review carefully these guidelines established by Health Canada as they effect your rights as a consumer:
It is important that every Canadian customer understand that the actions of Customs on the Canadian border are completely out of the control of Lifestyles Online Mall, LLC (LOM) and SupraLife/Front Runner Nutrition (SLI). Therefore, Canadian residents accept full responsibility for products ordered. LOM and SLI will do everything within their ability to ensure proper shipment, but cannot guarantee that Customs will not delay, withhold, return or confiscate the products shipped into Canada.
All purchases made by customers in Canada must be for personal use ONLY. There can be no resale of SupraLife products by Canadian customers.
In addition, each person wishing to use the products must place their own separate order (including family members).
All shipments must be made to a Canadian customer's residence directly. No shipment can be made to a business address or post office box number.
A Canadian customer is limited in the amount they can order and how often as follows:
- No more than a 3 month supply may be purchased at one time; and
- No more than one order can be placed each quarter (thus every 3 months).
By placing your order you acknowledge that you have reviewed and understand the foregoing guidelines and will abide by these rules established by Health Canada for a "personal use" exemption.
